Form No. 03 is a document to explain why your company needs foreign workers. You use this form when you apply for a new work permit, renew a work permit, or extend a work permit. How to fill in Form No. 03 for work permit application
To make sure your application is accepted quickly, follow these instructions carefully:
(1) Write the name of the government office that will process your work permit application.
(2), (10) Write your company type. Choose from: state sector, enterprise (state-owned, private, or foreign-invested), non-state public service unit, foreign sector, contractor, or other organization.
(3), (11), (12) Write your complete address in this order: house number and street name, hamlet or village, commune or ward, district, province or city.
(4) Write your economic sector code (level 1) according to Decision No. 27/2018/QD-TTg dated July 6, 2018.
(5) Write your business activities according to Vietnam’s conditional business list.
(6) Write the worker’s position clearly: manager, executive director, expert, or technical worker.
(7) Write the job name using the level 3 occupation code from Decision 34/2020/QD-TTg dated November 26, 2020. This decision lists 242 official job categories in Vietnam.
(8) Write the work arrangement type according to Clause 1, Article 2 of Decree 219/2025/ND-CP.
(9) Write the education level if the worker has one: doctoral degree, master’s degree, bachelor’s degree, or other qualification.
You do not need to fill in this form if:
- Your worker is a foreign contractor
- Your worker is exempt from work permits (example: experts working less than 90 days per year)
- Your worker transfers within the same company
